The Post Psychiatric Hospitalization Visit: Efficient Interventions for Primary Care Providers

With the current state of increasing mental health care needs in Arizona’s youth, pediatric primary care providers are often called upon to manage increasingly complex mental health concerns.  One of the most complex situations for a primary care provider is how to best help manage a pediatric patient who has been discharged from a brief inpatient psychiatric hospitalization.  Often times these patients come to clinic with little documented information, confusion about their medications, and frustration surrounding their experience in the hospital. 

These visits require a tremendous amount of energy, skill, and organization to best help the patient and their family system. These visits usually require more time than allotted, and can impact work flow and mental load for the provider for the remainder of the day.  Learning how to prepare for and practically manage these visits when they arise can help improve patient care and reduce provider burn out. Join this presentation to learn how a post psychiatric hospitalization protocol can help you set your patients up with a plan for their mental stability and safety.

At the conclusion of this activity, learners will be able to: 

  • Identify practical steps to effectively manage pediatric patients who are experiencing a resolving mental health crisis
  • Differentiate how to prioritize between medication interventions and psychosocial interventions in the post psychiatric hospitalized patient
  • Integrate elements of proposed post psychiatric hospitalization protocol into current work flow of outpatient pediatric primary care practice

Dr. Yoblonski has worked at Phoenix Childrens Hospital in the Homeless Youth Outreach program and the Center for Resiliency and Wellbeing for the past eleven years. She currently works with homeless youth and families, foster children and youth, and survivors of sex trafficking. Trauma informed care is an area of interest due to the patients she works with. She has been working with the AzAAP Emotional and Behavioral Health Committee for the past two years.

Dr. Shivana Naidoo, MD is a board certified Child Psychiatrist and a board certified Adult Psychiatrist.  She is the founder of DoBetterMD, her integrative psychiatry private practice and consulting firm, which aims to help patients with mental health concerns do better.  She is an Attending at the Bradley REACH Virtual Partial Psychiatric Hospitalization Program where she cares for high risk youth in a virtual multidisciplinary team setting. She is also the host of the podcast  "Thinking It Through with Dr. Naidoo- Child Psychiatrist", which helps spread awareness and psychoeducation  to help youth and their families find the best mental health care possible.
